Include: Mail or email the completed application form along with the other required supporting documents Requirements: Must be of African American descent, with at least one African American parent; Must reside in Mahoning, Trumbull or Columbiana County; Cumulative GPA of 2.5 or higher; Plan to pursue a four-year post-secondary degree or a two-year trade program; Application form must be fully completed and signed; High school transcript must be signed by school Principal or Guidance Counselor; Three letters of recommendation from non-family members.
